As a client, how can I authorize someone to view my ComForcare Report™?
The client receiving services must sign a release form for any individual wanting access to the ComForcare ReportTM. Along with the release, we will need the authorized user's name, address, phone number, relationship to client and email address.
What happens after you receive my authorization?
We will contact the authorized user and inform them about the ComForcare ReportTM request. If they would like to access the report online, we will create an account for the client and email the ComForcare ReportTM website address, User ID and password to the authorized user. Otherwise, the authorized user may choose to have the report mailed.
How will the recipient know when an updated ComForcare ReportTM is available?
An automatic email notification will be sent when a new ComForcare ReportTM is available. Each monthly report will be available by the middle of the month following the reporting period. Any authorized user that has requested a mailed report will automatically receive the report by mail during that time.
How much is the ComForcare Report?
The ComForcare ReportTM is included with services at no extra charge. It is completely free for our clients.
